Support Coordination
Soul Support Coordination aims to improve health and wellbeing outcomes for NDIS participants by helping them to participate more fully within their community. I do this by providing one on one virtual service delivery throughout Australia regardless of location, making quality support and care more accessible.
For those people who live on the Sunshine Coast, I can visit with you face to face in your home or in the community.
If support coordination is included in your NDIS plan, then you are already eligible to get assistance to not only understand the process, but to implement your plan the best way possible.
I can support you whether you are accessing the NDIS for the first time or if you have been a participant in the scheme for some time.
I do not discriminate and so support all Australians by offering people with disabilities guidance, and in the process helping them to learn new skills, building their capacity to become more independent and confident.
Specialist Support Coordination
Specialist support coordination is a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. As a specialist Support Coordinator, I can assist you to manage challenges in your support environment and by ensuring consistent delivery of service.
I work with people, their families, and carers to identify and understand the barriers impacting a person’s ability to access, use and maintain their supports.
It is my responsibility to find solutions to address those barriers and establish a process to overcome those barriers, including prioritising factors that need to be addressed first and what supports need to be put in place immediately.
Design a service plan for complex support needs, where appropriate, sometimes a participant may need a service plan to address significant complexities in their life.
Specialist support coordinators should work with the participant, their families, and carers to identify a broad network of supports that can help the participant use their plan.
Specialist support coordinators should design a service plan which:
- Ensures all services/providers can work effectively and cohesively together to help a participant to achieve their goals.
- Establishes processes to enable communication, cooperation, and collaboration within the participant’s network of funded and non-funded supports.
- Helps the participant, their family and carers to resolve issues and unexpected situations.
- Ensures a consistent delivery of service and access to relevant supports in a crisis or unplanned event.

Psychosocial Recovery Coaching
Psychosocial Recovery Coach, is a little different to support coordination. A PRC is a specialist mental health support person that some NDIS participants with mental health conditions may have access to under their plans.
This role is designed to support you when your mental health needs mean’s that you need increased levels of support. This role is also designed to support individuals to recognise, reflect and take responsibility for their own recovery.
As with support coordination, a PRC will help you identify any barriers that may have been getting in your way and use a strengths-based approach to support you to overcome them.
I will work collaboratively with your family, carers and other services to design and implement a recovery plan and create some relapse prevention strategies to ensure that you do not go backwards in your recovery journey.
As you can see, Psychosocial Recovery Coaching is quite similar to the role done by a Support Coordinator, but with the added benefit of being available outside of traditional office hours (the hours will be dependent on what the NDIA has allocated to you).
I understand that not everyone has the same wants and needs, so my commitment to you as your Support Coordinator of Psychosocial Recovery Coach, is that I will look objectivity at all of the options available, to find the best supports and options for you.

What is the NDIS?
The National Disability Insurance Scheme (NDIS) is a government funded scheme that aims to provide funding support for people living with a disability, their families, and Carers.
The scheme’s purpose is to financially support the people and their families in our community who have a disability. Only Australians under 65 years old, living with a permanent and significant disability, can apply for NDIS funding.
These disabilities include physical, intellectual, psychosocial, sensory, and cognitive impairment. Funding is also available as intervention supports for children with a developmental delay. The NDIS aims to help participants achieve their goals and live life to their fullest potential.
Enabling choice and using a person-centered approach ensures that the person with a disability is in complete control of all decision making in relation to their goals and all services.

How I Can Help to Manage Your Plan
Self-Managed Participants
Participants who are self-managing their plan are invoiced by providers and so they pay providers directly.
Participants will need a receipt from the provider to acquit the expenditure against their plan using the Myplace participant portal.
Plan-Managed Participants
Where a participant has a plan manager assisting them, a provider will need to send their invoices directly to the participant’s plan manager. Invoices must include a valid ABN unless the provider is exempt from quoting an ABN.
The plan manager will then process the payment through the myplace provider portal and pay the provider.
NDIS-Managed Participants
All registered providers will need to submit a payment request through the myplace provider portal to receive payment for their services or supports for participants who are NDIA-managed.
Changing Support Coordinators
I understand how incredibly important finding the right Support Coordinator is. Having someone you feel you can relate to, talk to, and trust will provide you with some much-needed peace of mind.
On top of this, knowing that they are qualified, experienced and that they understand how to navigate the NDIS is incredibly important also.
Did you know that a participant can choose to change support coordination providers (Coordinators) at any time, as long as the participant follows the notice periods in their service agreement with their current provider. The notice period is not usually very long, and most Support Coordinators are happy to release you from your service agreement early.

Get in touch and should you feel comfortable with your decision and wish to change, please let me know.

Nothing for you to do, as I will contact your current Support Coordinator to arrange the transition (handover notes etc). This will ensure there is absolute continuity, (no disruptions) to the delivery of your existing supports.

I will then contact the NDIS and all your relevant service providers to let them know of the change.

Have you been funded for Support Coordination ?
Choosing your Support Coordinator should be the first decision you make as soon as your plan is approved. Good support coordination has the biggest impact on getting the most out of your plan and making sure you achieve your goals.
It is important to choose someone you are happy to work with and feel you can trust. They should understand and explain all the support options available to you, whilst genuinely caring about your goals, dreams, and well-being.
They should also have time to listen and not have too many clients (participants), because they can’t provide adequate time for you.
Is there a conflict of interest? This maybe the case, if they are also providing other services to you, eg; support workers and or plan managers etc.

How support coordination gets included in a plan
Once the planning conversation is completed, your NDIA representative or LAC (Local Area Coordinator) will write up a plan to be approved. Once it has been approved and finalized, this first plan will be your entry point to the NDIS and the start of your relationship with the Scheme.
The funding that has been allocated to you, will be based on what the NDIS terms as being ‘‘reasonable and necessary’ for a person to pursue their short and mid to long term goals. This will be in addition to the support that may be provided by your family, friends, and other community and government services.
Where the NDIS has determined that it is reasonable and necessary for Support Coordination to be included in the Capacity Building budget. This will be a fixed amount for a Support Coordinator to help you implement your plan and put it into action.
As a participant, this funding will be for you to control and to choose how to use.
If your plan states the level (amount) of support coordination funded, then you can only purchase (use) this level of support coordination.
If your plan does not describe the level of support coordination funded, you can choose to purchase the level of support coordination that suits your needs.

WORKING TOGETHER
Whether you are a carer, a guardian and or nominee, you may be required to provide support and assistance to someone you care about who is affected by an illness and disability, so having a strong network of support can definitely aid in their recovery.
For some, because you often spend the most time with the person, you may be able to share some insight and ideas about what may be useful moving forward.
As a Support Coordinator, my relationship with you is incredibly important, as you have a right to be included in the work that occurs between a provider and the person, as it may have an impact on your life as well. Also, quite importantly, this might mean that you may require support for yourself if a family member or person you care about becomes unwell.
I continually work to improve my practice in working effectively with both the people with an illness and disability, but also their carers, as I recognise how important your role can be in a person’s life. Working together as a team where possible, so that everyone is clear about how to best support a person’s recovery.
Be assured that I have an absolute passion for making a difference in people’s lives, I am empathetic and understanding, but I also have immense knowledge and experience working as a Support Coordinator within the NDIS sector.
Support & Other Service Providers
As your Support Coordinator, I can help you make informed decisions about which support you would like in your life. Living in the Sunshine Coast area, I have a strong network of high-quality service providers who I can recommend because they are as equally passionate about helping people as I am.
If you do not live in the Sunshine Coast, be assured that a good proportion of my clients do not either. I have an excellent network of providers across Melbourne and other areas of Victoria at the moment, but as previously mentioned I can potentially support a participant anywhere in Australia. It is my job to ensure that they are not only reputable but are also a caring organisation.
Fees and Pricing
Support Coordination is a recognized capacity building – category of funds within your NDIS plan and as such, you may be able to access my service directly through your plan.
There are no hidden fees for my Support Coordination service.
There is no charge for the initial meet and greet. These meetings are about getting to know you and your family or carer’s to understand what you are looking for and how I can help you.
I charge as recommended by the NDIS and only for the time it takes to service the needs of my clients, nothing more.
